I, the above listed tenant, understand that by submitting this work order, I agree to the conditions as outlined in my rental agreement. I have followed applicable procedures to resolve the problem myself prior to submitting this request. I understand that submitting a work order does not guarantee all repairs will be made. In many cases, owners may have to approve estimates in writing before work can be completed. I also understand that if no problems are reported by the vendor and/or it has been determined that the problem was caused due to tenant damage and/or neglect, the tenant will bear the cost of repair. In case a vendor does not respond within 48 hours to a problem reported with heating and air conditioning, appliance, plumbing, and electrical problems, it is my responsibility to contact Rental Management Company.
